Fill out the Repair Order Form and BE SURE YOU PUT IT INTO THE BOX YOU ARE SHIPPING.

Carefully package enclosed sterilized handpieces along with your Repair Order Form.

Drop package at any UPS store, UPS Drop Box or call UPS for pick-up (go to UPS.com/dropoff for locations).
